Susan Moore
President
Susan Moore (President), RN, FCN, is a Specialty Dementia Navigator with Logan Health. She began her nursing career as an RN in Atlanta, GA. She and her husband of 40+ years then moved to Kalispell, MT, and she currently works at Logan Health for the last 8+ years as they build their ‘homestead’.
Susan has a diverse nursing background, including roles in Cardiac Stepdown, unit Supervisor, Inpatient RN Case Manager, a Faith Community RN, and most recently in her current role as the Specialty RN Navigator for the newly launched Logan Dementia Program. Susan is also RN Case Manager with a home care agency where she works with Veterans and MCD clients, many of whom live with dementia. All of these experiences have strengthened her desire to serve in promoting health and wellness, and collaborating with our community to enhance access to resources. She sees daily the health and life challenges our friends and neighbors face and is committed to compassionate person centered care and relationships.
Susan’s family is currently dealing with dementia and possible Alzheimer’s diagnosis for both of her parents, so this issue is close to her heart.
